Compensation Range: $18,000 – $65,000 per year

About the Role

OneCare Living Network is seeking compassionate and dedicated Host Home Providers across multiple locations in Colorado. This is a unique opportunity to provide meaningful, day-to-day support by welcoming an individual with intellectual and developmental disabilities into your home.

As a Host Home Provider, you offer a safe, stable, and supportive living environment for individuals who are unable to live independently but prefer a family-style setting over a group home. This role goes beyond caregiving—it’s about building relationships, creating a sense of belonging, and supporting individuals in living a full and independent life.

Key Responsibilities

• Provide daily personal care, including assistance with hygiene, meals, and routine activities

• Create a safe, structured, and supportive home environment

• Support individuals in developing life skills and increasing independence

• Encourage participation in community, social, and recreational activities

• Provide transportation to appointments, work, and community events as needed

• Assist with medical needs, including attending appointments and medication administration (as trained)

• Maintain accurate documentation, including progress notes and required records

• Integrate the individual into your home and daily life in a meaningful and respectful way

What We’re Looking For

• High school diploma or equivalent (required)

• Ability to pass criminal background and motor vehicle checks

• Willingness to complete required training (CPR/First Aid, Medication Administration, and ongoing training)

• Experience supporting individuals with developmental disabilities preferred

• Experience with behavioral or medical needs is a plus

• A compassionate, patient, and dependable approach to care

• A safe and stable home environment with a private bedroom available
