Compensation Range: $40– $60 per hour

Position Summary

OneCare Living Network Inc. is seeking a compassionate and experienced Registered Nurse (RN) – PRN to provide required nursing oversight for In-Home Support Services (IHSS) and other Home and Community-Based Services (HCBS) programs that require licensed nursing involvement. The RN ensures that services are delivered safely, effectively, and in compliance with state regulations, organizational policies, and person-centered care principles.

This role is ideal for a nurse who values flexibility, purpose-driven work, and making a meaningful impact in the lives of individuals and families.

Key Responsibilities

  • Provide clinical oversight and nursing consultation for IHSS and applicable HCBS programs

  • Conduct nursing assessments, evaluations, and documentation as required by state regulations and organizational policies

  • Collaborate with individuals, families, caregivers, and interdisciplinary teams to support person-centered care plans

  • Ensure health and safety needs are identified and addressed in a timely and professional manner

  • Support staff with clinical guidance, education, and best practices related to health and wellness

  • Monitor compliance with applicable policies, procedures, and regulatory requirements

  • Participate in care coordination with healthcare providers, case managers, and community partners

  • Maintain accurate, timely, and confidential clinical documentation

  • Serve as a clinical resource to support quality improvement and service excellence

IHSS-Specific Nursing Oversight Responsibilities

In accordance with IHSS requirements and applicable HCBS regulations, the RN will:

  • Provide clinical oversight for individuals receiving In-Home Support Services to ensure health-related needs are appropriately identified and addressed

  • Review and support health-related components of service plans, ensuring alignment with physician orders, care recommendations, and individual needs

  • Monitor and evaluate changes in health status, escalating concerns and coordinating care as needed

  • Provide guidance and oversight related to medication management, health maintenance, and wellness supports (within scope of practice and program requirements)

  • Serve as a clinical liaison between individuals, families, caregivers, case managers, and healthcare providers

  • Support compliance with state-mandated nursing involvement, documentation standards, and timelines for IHSS

  • Assist with staff education related to health monitoring, safety awareness, and recognition of changes in condition

  • Ensure nursing-related documentation is completed accurately, timely, and maintained in accordance with organizational policy and regulatory requirements

Minimum Qualifications

  • Current Registered Nurse (RN) license in the state of practice

  • Basic Life Support (BLS) certification for healthcare providers

  • Current driver’s license, insured and reliable transportation

  • Acceptable driving record

Preferred Qualifications

  • Ability to communicate effectively both verbally and in writing

  • Previous experience in Acute Care, Home Health, or Hospice settings

  • Bachelor’s Degree in Nursing (BSN)

Work Schedule

  • PRN / As-needed

  • Flexible scheduling based on program needs
